This is a working overview of placebo-controlled trial, written for readers who want more than a one-paragraph summary but less than a textbook.

Whether the behavioral effect originates centrally, peripherally, or through both remains an active question. Animal experiments using receptor antagonists and site-specific injections point toward hypothalamic melanocortin circuits as a key locus, but translating those findings to humans is not straightforward. Blood pressure changes observed in trials suggest a vascular component that may be peripherally mediated. The relationship between receptor occupancy and reported effect has not been mapped in humans, and no validated biomarker predicts response. This gap makes it difficult to explain individual variability on pharmacological grounds alone.
Bremelanotide acts as an agonist at melanocortin receptors, a family of five G-protein-coupled receptors labeled MC1 through MC5. Binding studies indicate activity at several of these subtypes rather than strict selectivity for one. Signalling proceeds mainly through Gs-mediated activation of adenylyl cyclase, raising intracellular cyclic AMP. The MC4 receptor, expressed in hypothalamic and limbic circuits, is widely regarded as the subtype most relevant to sexual response. Because the molecule is not subtype-selective, effects at other melanocortin receptors are expected and are used to explain some observed side effects.
The peptide contains seven amino acids arranged in a ring, closed by a lactam bridge between a side-chain acid and an amine. This cyclic constraint holds the backbone in a defined conformation and increases resistance to enzymatic breakdown relative to linear analogs. N-terminal acetylation and a C-terminal amide further protect the molecule from exopeptidases. The result is a compound with a comparatively long circulation time for a small peptide. Structural modification of the bridge alters receptor affinity, which is one reason analogs in this family differ in their subtype preferences.
Early clinical work used an intranasal formulation, and later programmes switched to subcutaneous delivery for more consistent absorption. A subcutaneous product received regulatory approval in the United States in 2019 for premenopausal women with acquired, generalised hypoactive sexual desire disorder. Approval followed phase 3 trials in which active treatment separated from placebo on desire and distress measures, though the average difference was modest. Labeling carries a caution about transient blood pressure elevation, so cardiovascular history is assessed before prescribing. Questions about durability of benefit beyond several months remain open.
Published discussion sits at the intersection of peptide chemistry, neuroendocrinology and sexual medicine. Trial reports emphasise change scores on validated instruments, while mechanistic papers focus on hypothalamic circuits and receptor selectivity. Because placebo response in this field is large, effect sizes are usually reported with confidence intervals rather than as isolated averages. Reviews note that female and male data sets are not interchangeable and should be read separately. Diagnostic terminology has been revised over time, which complicates comparison between older and newer studies.

Clinical research typically uses randomised, double-blind, placebo-controlled designs. The most common primary endpoint is the desire domain score of the Female Sexual Function Index, sometimes paired with a distress measure. Secondary outcomes include arousal, satisfaction, and event-based counts of satisfying sexual episodes. Across trials, average improvements are modest and individual responses vary widely. Whether benefits persist beyond a few months, and whether they depend on baseline hormone status, remain open questions rather than settled findings.
Melanocortin receptors form a family of five G-protein-coupled receptors designated MC1 through MC5. Bremelanotide binds most strongly at MC4R and MC1R, with weaker activity reported at MC3R and MC5R. MC4R is expressed in hypothalamic nuclei that coordinate energy balance and aspects of sexual behaviour. The prevailing interpretation is that central MC4R activation, rather than peripheral vascular effects alone, drives the reported changes in desire. This account remains partly inferential, since direct receptor-level measurement in living humans is not practical.
Clinical development proceeded through two routes of administration. An intranasal formulation advanced first, but variable absorption and tolerability problems led to a switch to subcutaneous injection. The United States Food and Drug Administration approved the subcutaneous product in 2019 for hypoactive sexual desire disorder in premenopausal women. Marketing rights subsequently changed hands, and commercial availability has fluctuated since approval. Use in men, in postmenopausal women, and in combination with other agents remains outside the approved label.
Outside the approved product, bremelanotide circulates as a research chemical sold by peptide vendors, often labelled PT-141. Such material is not manufactured under pharmaceutical quality standards, and independent testing has repeatedly found content that differs from the label. Analytical certificates supplied with a purchase are not strong evidence of purity because they are usually generated by the seller. Online discussion tends to blur the distinction between the approved drug and unregulated powder, which complicates interpretation of reported experiences.
The panel attributed many of the deaths to natural causes or substandard care, citing issues such as inadequate staffing and delays in essential treatment, and argued that the deaths were unrelated to deliberate actions by Letby. On 14 September 2026, Helen Shannon, a chemical engineering expert, and Geoff Chase, a professor of bioengineering, resigned from Letby's legal team, saying that some of the arguments being put forward on Letby's behalf regarding two infants were "inconsistent with the available evidence, science and established physiology".
In 1943, the Medical Research Council decided that the time had come for field trials of penicillin. The location of centres to receive the drug was kept secret so as to not provoke demand for the drug when it was still in short supply. Howard Florey was sent to North Africa, where the North African campaign was ongoing. On 29 June he was joined by Hugh Cairns, another Rhodes Scholar from Adelaide, who now held the rank of brigadier in the British Army, and was in charge of the Military Hospital for head injuries in Oxford, who brought with him a stockpile of 40 million units of penicillin. Over the next two months Florey and Cairns treated over one hundred cases and compiled a report that ran to over a hundred pages. They also filmed their activities. Florey gave lectures on penicillin, and his report contained recommendations for training of medical officers in its use. The fighting in North Africa had ended in May 1943, so most of the cases he saw were not recently wounded soldiers, but ones with old wounds that had not healed; battle casualties began arriving again after the Allied invasion of Sicily in July. Florey considered that the source of infection in many cases was the hospital rather than the battlefield, and advocated changes to the way that patients were treated to take advantage of the properties of penicillin. He argued that wounds should be cleaned and sealed up promptly. This was a radical idea; normally it would have been inviting gas gangrene, but he proposed leaving that to the penicillin.
This is a list of notable restaurant chains in Australia. A restaurant chain is a set of related restaurants with the same name in many different locations that are either under shared corporate ownership (e.g., McDonald's in the U.S.) or franchising agreements. Typically, the restaurants within a chain are built to a standard format through architectural prototype development and offer a standard menu and/or services.
Azurin's other domains may also exhibit strong anticancer activity by binding to cell surface receptor tyrosine kinases such as EphB2 receptors, which induce angiogenesis in cancer cells. This is another mechanism by which azurin has been proposed to exhibit its therapeutic effects.

=== Prosecution in the United States === Following his capture Noriega was transferred to a cell in the Miami federal courthouse, where he was arraigned on the ten charges which the Miami grand jury had returned two years earlier. The trial was delayed until September 1991 over whether Noriega could be tried after his detention as a prisoner of war, the admissibility of evidence and witnesses, and how to pay for Noriega's legal defense. The trial ended in April 1992, when Noriega was convicted on eight of the ten charges of drug trafficking, racketeering, and money laundering. On July 10, 1992, Noriega was sentenced to 40 years in prison. In pre-trial proceedings, the government stated that Noriega had received $322,000 from the U.S. Army and the CIA. Noriega insisted that he had in fact been paid close to $10,000,000, and that he should be allowed to testify about the work he had done for the U.S. government. The district court held that information about the operations in which Noriega had played a part supposedly in return for payment from the U.S. was not relevant to his defense. It ruled that "the tendency of such evidence to confuse the issues before the jury substantially outweighed any probative value it might have had." One of the witnesses in the trial was Carlton, who had previously flown shipments of drugs for Noriega. Information about Noriega's connections to the CIA, including his alleged contact with Bush, were kept out of the trial. After the trial, Noriega appealed this exclusionary ruling by the judge to the Eleventh Circuit Court of Appeals.
== Description == Leaves are aromatic, simple and lanceolate, obovate-elliptic or elliptic, size of 5–15 cm × 2–5 cm, coriaceous to thickly coriaceous. The leaves are 5–15 cm × 1.5–5 cm, apex acute, lower side pubescent. Flowers are solitary, bisexual, pink to dark red, axillary or subterminal. Flower peduncle size is 1.5–4 cm, tepals number range from seven to twelve, and are broadly elliptic to broadly ovate, anthers size is 1–1.5 mm, pollen grains trisyncolpate. The fruit is a capsule-like follicetum, star-shaped, reddish-brown, consisting of six to eight follicles arranged in a whorl. Each follicle is boat-shaped, 1–2 cm long, rough and rigid, color reddish-brown, with 1 seed, opening along the ventral edge when ripe. carpels size of 10 mm long, boat-shaped; they are hard and wrinkled, containing one seed. Seeds are brown, compressed ovoid, smooth, shiny and brittle with approximate size of 8–9 mm × 6 mm. Differences with similar taxa: Illicium anisatum had smaller fruits that does not form a regular star due to the abortion of some carpels. Also fruit follicles are not swollen in the middle and had a more pointed apex. Also usually had more than 8 follicles and the fruit has weaker odour. The seeds in Illicium anisatum are flat or almost spherical.
Valve enlisted Gearbox Software to develop three expansions for Half-Life: Opposing Force (1999), Blue Shift (2001) and Decay (2001). In 1998, Valve acquired TF Software, a group that had made the popular Team Fortress mod for Quake, and remade it for GoldSrc as Team Fortress Classic in 1999. Valve released the software development kit (SDK) for the GoldSrc engine, facilitating numerous user-created mods. They acquired the developers of one popular mod, Counter-Strike, to create a standalone Counter-Strike game. Happy with Valve's success, Harrington sold his stake in Valve to Newell in 2000. Valve's publishing agreement meant Sierra owned the Half-Life intellectual property and held exclusive publishing rights to future Valve games. In 2001, Valve renegotiated by threatening to cease game development and develop other software, using an offer of a partnership from Amazon to create a digital storefront as a bargaining chip. After the agreement with Sierra was amended, Valve gained the Half-Life intellectual property and online distribution rights for its games.
Masculinizing hormone therapy is typically used by transgender men, who desire the development of masculine secondary sex characteristics. Masculinizing hormone therapy usually includes testosterone to produce masculinization and suppress the production of estrogen. Treatment options include oral, subcutaneous injections or implant, and transdermal (patches, gels). Dosing is patient-specific, depending on the patient's rate of metabolism, and is discussed with the physician. The most commonly prescribed methods are intramuscular and subcutaneous injections. This dosing can be daily, weekly or biweekly depending on the route of administration and the individual patient. Unlike feminizing hormone therapy, individuals undergoing masculinizing hormone therapy do not usually require additional hormone suppression such as estrogen suppression. Therapeutic doses of testosterone are usually sufficient to inhibit the production of estrogen to desired physiologic levels. The desired effects of masculinizing hormone therapy focus on the development of masculine secondary sex characteristics. These desired effects include: increased muscle mass, increased bone turnover, development of facial hair, voice deepening, increase and thickening of body hair, and more.
There is extensive experience, over many years, of the use of radioiodine in the treatment of thyroid overactivity, and this experience does not indicate any increased risk of thyroid cancer following treatment. However, a study from 2007 has reported an increased number of cancer cases after radioiodine treatment for hyperthyroidism. The principal advantage of radioiodine treatment for hyperthyroidism is that it tends to have a much higher success rate than medications. Depending on the dose of radioiodine chosen, and the disease under treatment (Graves' vs. toxic goiter, vs. hot nodule, etc.), the success rate in achieving definitive resolution of the hyperthyroidism may vary from 75 to 100%. A major expected side-effect of radioiodine in people with Graves' disease is the development of lifelong hypothyroidism, requiring daily treatment with thyroid hormone. On occasion, some people may require more than one radioactive treatment, depending on the type of disease present, the size of the thyroid, and the initial dose administered. People with Graves' disease manifesting moderate or severe Graves' ophthalmopathy are cautioned against radioactive iodine-131 treatment, since it has been shown to exacerbate existing thyroid eye disease. People with mild or no ophthalmic symptoms can mitigate their risk with a concurrent six-week course of prednisone. The mechanisms proposed for this side effect involve a TSH receptor common to both thyrocytes and retro-orbital tissue.

It is frequently described as an MC4 receptor agonist, but binding assays show activity at more than one melanocortin subtype. Selectivity is therefore relative rather than absolute. This matters when interpreting side effects tied to other receptor subtypes.
The lactam bridge locks the peptide into a defined shape and makes it harder for peptidases to cut the backbone. Terminal blocking groups add further protection at both ends. Together these features extend the molecule's persistence in circulation compared with linear peptides.
Evidence from animals favors hypothalamic melanocortin circuits, yet direct confirmation in humans is lacking. A peripheral vascular contribution has also been proposed. How receptor binding translates into a reported behavioral effect is still not mapped.
It is a synthetic cyclic peptide that activates melanocortin receptors. It is given by injection and was approved in the United States in 2019 for a specific low-desire diagnosis in premenopausal women. It is not a hormonal therapy.
